Alumni Award Nominations

Nomination Deadline:  Friday, January 19, 2018

UConn Agriculture, Health and Natural Resources Alumni is currently accepting nominations for the annual UCAHNRA Awards. You are invited to nominate deserving alumni. Criteria and required nomination material for each award is noted below.

All award recipients will be recognized at the annual CAHNR Awards and Honors Event on March 28, 2018.

Distinguished Alumni Award

Criteria for nomination:
  • Alumni status from the UConn College of Agriculture, Health and Natural Resources or Ratcliffe Hicks School of Agriculture
  • Demonstrated excellence in the award category
  • Significant contributions to her/his professional environment; outstanding reputation for his/her work and has been an inspiration to others
  • Professional and/or community service
Nomination Requirements:
  • A letter from the individual making the nomination to include name, year of graduation, degree and major, and description of the significant qualifications, abilities, characteristics and achievements that make this nominee an outstanding candidate for the award. Self-nominations accepted.
  • A copy of the nominee’s curriculum vitae or resume.
  • Two additional letters of support will be accepted but are not required.
  • Optionally, submit any additional supporting documentation that will assist the selection committee in evaluating the nominee’s suitability for an award, e.g. publications, previous awards, news articles, etc.

Excellence in Athletic Training Award

Criteria for Nomination:
  • Must be a graduate of the UConn Athletic Training Program
  • Must show evidence of outstanding clinical care, professional service, education, community or scholarly activity
  • Preferred to have evidence of consistent contributions to the profession including recognition and honors in endeavors related one or more areas of clinical care, education, service, and scholarly activity
Nomination Requirements:
  • A brief cover letter from the primary nominator that clearly states the category of the nomination and the impact area from selection criteria listed below.
  • A letter of endorsement from UConn faculty or staff.
  • No more than two (2) additional letters in support of the nomination from persons with an intimate knowledge of the nominee’s accomplishments. 
  • The nominee’s curriculum vitae (maximum of five pages).
  • Optionally, submit any additional supporting documentation that will assist the selection committee in evaluating the nominee’s suitability for an award, e.g. publications, previous awards, news articles, etc.
Candidates will be evaluated on the basis of demonstrated excellence and lasting impact in one or more of the following areas:
  • Clinical expertise and commitment to health care within a community or specific athletic population
  • Service with meaningful impact on the Program, University, and/or profession
  • Development and delivery of health care education
  • Scholarly activity as evidenced by grants, publications, and professional reputation
  • Creativity or innovation in health care education, research or implementation of athletic training services

Excellence in Exercise Science Award

Criteria for Nomination:
  • Must be a graduate of a UConn Exercise Science program.
  • Must show evidence of outstanding performance in an area of Exercise Science with regards to clinical activity, professional service, education, community or scholarly activity
  • Preferred to have evidence of consistent contributions to the profession including recognition and honors in endeavors related to one or more areas of health and wellness, education, service, and scholarly activity
Nomination Requirements:
  • A brief cover letter from the primary nominator that clearly states the category of the nomination and the impact area from selection criteria listed below.
  • A letter of endorsement from a UConn faculty or staff member
  • No more than two (2) additional letters in support of the nomination from persons with an intimate knowledge of the nominee’s accomplishments. 
  • The nominee’s curriculum vitae (maximum of five pages).
  • Optionally, submit any additional supporting documentation that will assist the selection committee in evaluating the nominee’s suitability for an award, e.g. publications, previous awards, news articles, etc.
Candidates will be evaluated on the basis of demonstrated excellence and lasting impact in one or more of the following areas:
  • Exercise science expertise and commitment to health and wellness within a community or specific population
  • Service with meaningful impact on the Program, University, and/or profession
  • Advancement of Exercise Science theory and practice
  • Development and delivery of Exercise Science education
  • Scholarly activity as evidenced by grants, publications, and professional reputation
  • Creativity or innovation in Exercise Science education, research or implementation of Exercise Science principles

Excellence in Physical Therapy Award

Criteria for Nomination:
  • Must be a graduate of the UConn Physical Therapy Program.
  • Must show evidence of outstanding clinical care, professional service, education, community or scholarly activity
  • Preferred to have evidence of consistent contributions to the profession including recognition and honors in endeavors related to or more areas of patient care, education, service, and scholarly activity
Nomination Requirements:
  • A brief cover letter from the primary nominator that clearly states the category of the nomination and the impact area from selection criteria listed below.
  • A letter of endorsement from a UConn faculty or staff member
  • No more than two (2) additional letters in support of the nomination from persons with an intimate knowledge of the nominee’s accomplishments. 
  • The nominee’s curriculum vitae (maximum of five pages).
  • Optionally, submit any additional supporting documentation that will assist the selection committee in evaluating the nominee’s suitability for an award, e.g. publications, previous awards, news articles, etc.
Candidates will be evaluated on the basis of demonstrated excellence and lasting impact in one or more of the following areas:
  • Clinical expertise and commitment to health care within a community or patient population
  • Service with meaningful impact on the Program, University, and/or profession
  • Development and delivery of health care education
  • Scholarly activity as evidenced by grants, publications, and professional reputation
  • Creativity or innovation in health care education, research or implementation of physical therapy services
